   (If you are purchasing this ticket as donation or gift to someone else, please include that, as well as their name, in the "Reason for Donation" field.)
   
   
3. Choose the correct amount:
   $345 - Deposit payment
   $645 - Full payment
   
   $300 - Remaining balance
   $350 - Full payment for those that have taken the first Rethinking Sickness program
